Transit Employees' Health and Benefit Trust

Instructions for Short Term Disability (STD) Benefits applications for Unifor 111 & 2200 employees.

Temporary Sick Leave Process for employees booking off sick due to a positive COVID19 result.

  • If the Plan Member has tested positive for COVID19 and absence is =<10 days, the following forms/documents must be submitted to HR Benefits:

     

    • Disability Claim Form (DCF)
    • Plan Member Confirmation of Illness Form (see attach form)
    • Confirmatory documentation substantiating a positive covid test (APS is not required to be completed by the doctor)
       
  • If the Plan Member has tested positive for COVID19 and absence is >10 days, the following forms/documents must be submitted to HR Benefits:
    • Disability Claims Form (DCF)
    • Attending Physician Statement (APS)
    • Confirmatory documenation substantiating a positive covid test

What constitutes "confirmatory documentation substantiating a positive covid test":

  • A copy of the email from Heatlh Gateway that shows they tested positive or
  • A screen shot of the text that they received from Health Gateway that shows they tested positive or
  • A photo of the rapid test kits result that shows they tested positive

Just a reminder that all forms and documents should be sent to HR Benefits at S725.
